I ELECTRONIC PATIENT CARE REPORTING

NexPCR. The patient record, captured at the point of care.

NexPCR gives field providers a fast, mobile-first way to document patient encounters — intelligent automation producing clean, standards-aligned records that flow straight into the hospital chart with no re-entry required.

THE PROBLEM NEXPCR SOLVES

Less paperwork. Better records. Smoother handoffs.

Paper-based and legacy ePCR systems slow crews down when every minute matters. Incomplete or delayed documentation creates gaps in the clinical record that follow patients into the hospital. NexPCR replaces that friction with a mobile-first, standards-aligned documentation system designed for the pace of pre-hospital care.

  • Mobile-first documentation. Built for tablets and phones so crews capture care in real time — not after the shift, not from memory — with an interface designed for gloved hands and high-stress environments.

  • NEMSIS 3.5 compliance. Every field maps to the national standard for EMS data, with built-in validations that catch errors before submission — eliminating manual reformatting for state reporting.

  • Offline capability. Full documentation functionality even without connectivity — records sync automatically when the device reconnects, so rural and low-signal environments don't compromise data quality.

  • Bidirectional NexEMS integration. Dispatch context from NexEMS pre-populates key PCR fields, and completed reports update the response record — closing the loop without duplication.

  • Automatic AXIA Medical sync. Completed PCRs post directly to the patient record in AXIA Medical so the receiving team sees the full clinical picture before the patient walks through the door.

  • Supervisor QA workflows. Structured review queues let supervisors flag, annotate, and sign off on PCRs from a central dashboard — improving crew accountability and documentation quality over time.

I THE DOCUMENTATION WORKFLOW

The record travels with the patient, from dispatch to discharge.

When NexEMS creates a dispatch record, NexPCR opens a pre-populated PCR on the crew's device, with incident number, dispatch time, unit ID, and call type already filled in. The crew adds scene findings, vitals, interventions, and medications as care is delivered, using structured forms built for fast entry in moving vehicles.

Built-in validations check for NEMSIS compliance and flag missing fields before submission. At handoff, the finalized record syncs to AXIA Medical automatically, giving ED staff the full pre-hospital picture without a verbal report or faxed form.

Afterward, supervisors review completed PCRs in their QA queue. Aggregate data feeds quality dashboards, and billing-ready exports are available once the record is approved.

Documentation that keeps up with care.
